Digital Consent Forms for Clinics & Therapists: The Client Intake & Medical Forms Powerup
"If you run a clinic, salon, or therapy practice, you know how essential intake forms are — and how much of a pain paper forms can be. The Client Intake & Medical Forms powerup brings them online, directly into your booking flow."
If you're a beautician, aesthetic practitioner, physiotherapist, personal trainer, massage therapist, or any kind of service professional, intake forms aren't optional — they're fundamental to your practice.
They protect your clients by ensuring you understand their health background before a treatment.
They protect you by creating a documented record of client consent and disclosure.
And yet, in most small practices, they're still handled with a clipboard and a biro that runs out halfway through the form.
The Client Intake & Medical Forms powerup changes this.
What It Does
The powerup adds a customisable digital form to your booking flow on Whizzly. When a client books a service through your storefront, they're prompted to complete the intake form before their appointment is confirmed.
Forms can include:
Free-text fields: Name, address, date of birth, emergency contact
Medical history checkboxes: Pre-populated with common categories (skin conditions, allergies, medications, surgical history, pregnancy status) — you choose which are relevant to your practice
Consent declarations: Tick-to-confirm statements with your custom wording (e.g., "I confirm I have disclosed all relevant medical conditions and understand the treatment risks as explained to me")
Digital signature: A finger/stylus-drawn signature captured on the client's phone — legally equivalent to a wet signature for most consent purposes in the UK
File/photo upload: For practitioners who need clients to submit ID, a GP referral letter, or before-photos
Compliance & Storage
All submitted forms are stored securely in your Whizzly dashboard, associated with the client's booking record. You can access, download, or print any form from the Orders section.
Forms are stored for a minimum of 7 years, in line with typical UK clinical record retention guidelines.
We recommend you consult with your professional indemnity insurance provider or governing body about their specific requirements for your discipline — requirements vary between, say, aesthetic practitioners and sports massage therapists.
Pre-Appointment or In-Clinic?
You can configure when the form is presented:
- Before booking is confirmed: The client must complete the form for their booking to be accepted. Best for practices where the form genuinely gates treatment (e.g., aesthetics)
- After booking, before appointment: The client receives a WhatsApp link to complete the form at their convenience. Best for longer, more detailed forms
- On arrival (QR code): A QR code posted in your reception area that clients scan to complete the form on their phone in the waiting room. Best for walk-in clients
